This is often how problems with a damaged skin barrier begin – not with one obvious symptom, but with a handful of small signs that are easy to blame on the weather or on ‘sensitive skin’.

You won’t find a repair plan here – I’ve set it out in a separate article, how to repair your skin barrier step by step. Several other conditions cause similar signs, and each one needs something different. This table will help you get your bearings – a diagnosis always comes from a dermatologist.

CONDITION

TYPICAL SIGNS

WHAT SETS IT APART

WHAT TO DO

Damaged skin barrier

Tightness after cleansing, stinging from products that never stung before, rough texture, flaking

Develops gradually, often after a change in habits: a new acid or retinol, longer cleansing, the heating season

Simplify your routine and repair your skin barrier

Sensitive skin

Burning, stinging or itching in response to things that shouldn’t cause it

Skin can look completely normal [1]

Limit triggers and choose hypoallergenic skincare

Contact allergy

Itching, bumps, swelling, sometimes small blisters

Appears after a specific product, where it was applied

Stop using the product; patch testing with a dermatologist

Rosacea

Redness across the centre of the face, visible blood vessels, flushing, bumps without blackheads

Redness worsens with triggers: heat, sun, alcohol, spicy food

A dermatologist and gentle skincare

Atopic eczema

Intense itching, dry, thickened patches, often on the eyelids and in skin creases

Long-term and recurring, often since childhood

Treatment with a dermatologist, emollients

Seborrhoeic dermatitis

Red patches with greasy scales on the sides of the nose, eyebrows and along the hairline

Appears where there are many sebaceous glands

A dermatologist

These conditions aren’t mutually exclusive. In atopic eczema, a weakened barrier is part of the condition itself [2], which is why barrier care goes alongside medical treatment, not instead of it.

What happens in your skin when the barrier is damaged?

Your skin barrier isn’t a layer of moisturiser or a film of sebum – it’s the outermost part of the epidermis, the stratum corneum. Picture a brick wall: flattened, dead cells are the bricks, and the lipids between them – ceramides, cholesterol and free fatty acids – are the mortar. Inside the bricks sits NMF (Natural Moisturising Factor), which binds water. I explain this structure in more detail in my article on how to repair your skin barrier.

When the mortar stops being watertight, two things happen at once. Water evaporates from your skin faster – this is called TEWL (transepidermal water loss). And in the other direction, irritants get in more easily: surfactants, fragrance allergens and ingredients that did your skin no harm while the barrier was intact. That’s where the two groups of symptoms come from: dryness and tightness on one side, stinging and reactivity on the other.

Your skin tries to defend itself. When the barrier is disrupted, the epidermis speeds up cell division [3]. The trouble comes at the very last stage: in a dehydrated stratum corneum, the bonds between dead cells break down less efficiently – and it’s these bonds breaking that lets cells shed evenly and invisibly [4]. Instead, the cells build up on the surface as dry flakes and rough patches. In my work I see that dry skin, or skin with a damaged barrier, takes longer to regain its balance than the textbook skin renewal cycle suggests – usually more than a month, sometimes two.

Damaged, compromised or broken skin barrier?

In scientific papers, you’ll read about impaired barrier function. ‘Damaged’, ‘compromised’ and ‘broken’ are everyday terms that describe the same problem – at most to a different degree. ‘Compromised’ tends to be used when skin feels tight and reactive but looks almost normal. ‘Damaged’ or ‘broken’ come up more often once there’s flaking, cracking or stinging with every product.

Signs and symptoms of a damaged skin barrier

The signs rarely appear one at a time. The more of the ones below you recognise, the more likely it is that the problem is your barrier rather than a particular product.

Tight skin after cleansing

This is usually the first sign. Water and surfactant wash out lipids and NMF, and a leaky barrier can’t replace them fast enough. If the tightness only goes once you’ve applied moisturiser and comes back every time you wash, take a closer look at what you cleanse with and how – I cover this in my article on skin that feels tight after washing.

Stinging from products that never stung before

The product hasn’t changed – your skin has. Through a leaky barrier, ingredients reach the nerve endings more easily, so your skin responds by stinging even to a neutral moisturiser. Stinging that fades within a few minutes is a typical reaction of a leaky barrier. Itching, bumps or swelling point to an allergy instead – I explain how to tell them apart in stinging or allergy?

Dryness, rough texture and flaking

This comes from the shedding process described above, which stops working evenly. Dead cells stay on the surface, your skin feels rough to the touch, and make-up clings to dry patches or pills on your cheeks and around your nose.

Dull, grey-looking skin

A smooth stratum corneum reflects light evenly. An uneven surface covered in built-up cells scatters it, so your complexion looks tired even after a good night’s sleep.

Redness and reacting to cold, wind and heat

Skin with a damaged barrier flushes in response to things it used to tolerate without a problem: stepping out into the frost, a warm room, a hot shower. If the redness stays in the centre of your face and you can see small blood vessels, go back to the table above – it may be rosacea.

Oily and tight at the same time

Oily skin can have a damaged barrier too. Sebum is made in the sebaceous glands and coats the surface of the skin, while barrier lipids are made by the epidermis and fill the spaces between cells. They are different fats with different jobs. In a study on mice without sebaceous glands, skin hydration dropped while the barrier kept working normally [5]. In people with acne, researchers found fewer ceramides in the stratum corneum and a weaker water barrier – and, in moderate acne, more sebum as well [6].

That’s why a shiny T-zone and tight cheeks can go hand in hand. You’ll often read that skin ‘produces more sebum to make up for missing lipids’. Research doesn’t support this: how much sebum you produce depends mainly on hormones and genes, and the T-zone is exactly where sebaceous glands are most numerous [7]. If anything, the relationship can run the other way – in a study on mouse skin, excess sebum worked its way between the barrier lipids and loosened their structure, and water loss increased [8].

I know this from my own skin. I had combination skin leaning towards oily, and I was still dealing with acne in my thirties. For a long time I looked after it badly: I mattified it with toners and masks for oily skin, and instead of a proper protective moisturiser I used a ‘light’ one that probably just dried it out. Things changed when I switched to gentle skincare and an oil-based cream. The amount of sebum decreased gradually, and for several years now my skin hasn’t been oily and rarely gives me any trouble. It isn’t down to age – in women, sebum production stays at a similar level from the end of the teenage years until the menopause [9]. I’m not claiming one cream changed how my sebaceous glands work. But I’m sure of one thing: gentle skincare did more for my skin than years of fighting sebum.

More spots and small bumps

Irritants and microbes get through a leaky barrier more easily. For some people, that means breakouts – more on this in the section on spots below.

What does a damaged skin barrier look like?

Most often you’ll see dull, uneven skin with dry flakes on the cheeks and around the nose and mouth, sometimes with diffuse redness. Some of the signs are invisible, though: you feel the tightness and stinging before you see anything in the mirror.

Can you test your skin barrier at home?

Not in the sense of measuring it. Barrier function is measured with a device that records TEWL – the rate at which water evaporates from the skin – which is how we test our own products, for example. Popular ‘home tests’ such as ‘wash your face and wait half an hour’ are simply observing symptoms, not a diagnosis. Observation still makes sense if you do it carefully: for a week, note how your skin reacts after cleansing, after your usual moisturiser and after going outside. If the tightness and stinging come back every day, look for the cause in your barrier, not in a single product.

Sensitive skin isn’t always a damaged skin barrier

‘I have sensitive skin, so my barrier must be damaged’ – it’s a shortcut I hear a lot. An international group of experts defined sensitive skin by how it feels: burning, stinging, itching, tingling or pain in response to things that normally shouldn’t cause them. The skin looks completely normal or slightly red, and no skin disease explains the symptoms [1]. There isn’t a single word about the barrier in that definition.

In practice the two often overlap, but they don’t have to. You can have sensitive skin with an intact barrier. You can also have a damaged barrier without any stinging – in that case you’re more likely to notice dryness, rough texture or oiliness. So if your skin still stings at everything after several weeks of consistent skin barrier repair, the cause may lie somewhere other than the barrier.

With sensitive skin, what matters most is limiting triggers: fewer products, shorter ingredient lists, no fragrance. This is where a category that, unlike ‘dermocosmetics’, requires additional testing makes sense – hypoallergenic skincare.

What damages your skin barrier? The most common causes

A barrier is rarely damaged by one thing. Most often it’s the sum of everyday habits and conditions, each of which seems harmless on its own.

Cleansing: too long, too hot, too harsh

Every wash removes some lipids and NMF from your skin, so it matters not only what you cleanse with but also how long and how often. Sixty seconds is enough to cleanse your face. Longer methods, such as the popular 4-2-4 rule, don’t give a better result – they simply prolong your skin’s contact with surfactant and water. The same goes for double cleansing. Wash your face morning and evening, but only once each time. If you wear heavy make-up, dissolve the excess with micellar water, then wash your face once.

Never leave micellar water on your skin – always rinse it off. It’s water with surfactants – milder than in many gels, but surfactants all the same. If you don’t rinse it off, the water evaporates and the surfactant stays on your skin, dissolving barrier lipids for hours. Preservatives and fragrance ingredients stay behind with it too, and they can irritate.

Water temperature matters as well. In a study by Berardesca and colleagues, the same surfactant (SLS) irritated the skin more the warmer the water was [10]. Wash your face with lukewarm water and pat your skin dry by pressing the towel to it a few times until it absorbs the water, rather than rubbing. I explain which surfactants are the most drying in my article on skin that feels tight after washing.

Hard water

Water with a lot of calcium makes surfactant stay on your skin. In a study by Danby and colleagues, washing with hard water left significantly more SLS on the skin, which increased TEWL and irritation – most of all in people with atopic eczema and a filaggrin mutation. Softening the water reduced this effect [11].

I learnt this the hard way. After moving somewhere with very hard water, I developed rosacea – the redness appeared literally the next day. If you live in a hard-water area, choose cleansers with a chelating agent, which binds calcium and magnesium ions.

Too many actives

AHA and BHA acids, retinoids, scrubs and sonic brushes speed up exfoliation. Use them too often, or several at once, and your skin loses its stratum corneum faster than it can rebuild the lipids between the cells. I explain how to combine actives so this doesn’t happen in my guide to retinol, vitamin C and niacinamide. Some medicines applied to the skin also weaken the barrier, such as topical steroids [12] – but don’t stop that kind of treatment on your own; talk to your doctor or dermatologist.

Fragrance and essential oils

Through a leaky barrier, allergens get deeper into the skin and sensitise it more easily. This applies to essential oils in natural cosmetics too – they contain the same fragrance allergens that manufacturers must list in the ingredients, such as Linalool or Limonene. ‘Natural’ doesn’t mean ‘gentle’ – I write about this in 5 myths about natural cosmetics.

Cold, dry air and the sun

Low humidity and low temperatures weaken barrier function and make skin more prone to irritation [13]. That’s why problems often start in the heating season, and a humidifier in your bedroom is real help rather than a gadget. UV radiation, in turn, weakens the cohesion of the stratum corneum – the bonds between cells and the lipids that hold them together [14]. So daily sun protection protects your barrier too.

Stress and lack of sleep

In students during exam season, the barrier recovered more slowly than after they came back from holiday [15]. In another study, both a stressful interview and a single sleepless night delayed barrier recovery in healthy women [16]. If you notice your skin ‘falling apart’ during difficult weeks, it’s no coincidence.

Age and genes

Older skin loses its barrier function more easily and regains it more slowly. In a study by Ghadially and colleagues, the barrier of people over 80 was easier to disrupt than in people aged 20–30, and it took longer to recover [17]. Genes matter too, especially filaggrin – a protein that hydration and barrier function in the stratum corneum depend on. In skincare guides you’ll read that ‘up to 50% of Europeans’ carry a filaggrin mutation. The study that described these mutations puts it at around 9% of people of European origin [2]. That’s still a lot – but not one in two.

What about diet?

Almost every guide lists diet among the causes of a damaged skin barrier. It does matter, but only in its extreme form: a severe deficiency of essential fatty acids, especially linoleic acid, which is a building block of ceramides, damages the barrier so clearly that researchers use it as a model of barrier disruption [3]. This kind of deficiency occurs, for example, in people fed intravenously for a long time with a fat-free formula, or with conditions in which the gut can’t absorb fat, such as cystic fibrosis or short bowel syndrome. An ordinary diet, even one low in fat, almost always provides linoleic acid – there’s plenty of it in vegetable oils, nuts and seeds. There’s no strong evidence that a particular food, supplement or drinking more water will repair the skin barrier on your face. If you’re looking for the cause, start with what you do to your skin every day – cleansing, water and cosmetics.

Damaged skin barrier and spots

Acne and a damaged barrier often go hand in hand, even though at first glance they’re two different problems. In a study by Yamamoto and colleagues, people with acne had fewer ceramides in the stratum corneum and a weaker water barrier than people without acne. The authors suggest that a lack of ceramides may encourage excessive keratinisation at the opening of the hair follicles – and so the formation of blackheads and whiteheads [6].

It works the other way too: the way you fight spots can weaken your barrier further. Harsh degreasing, alcohol-based toners, mattifying masks and several acids at once are the classic combination that leaves skin dried out and still oily. I went through this myself, as I describe above. If you’re treating acne with a medicine prescribed by your dermatologist, don’t stop it – support your barrier with gentle cleansing and a moisturiser instead. I set out the basics of daily care in my article on an acne-prone skin care routine.

What to do if you recognise these signs

The first step isn’t buying a new cream – it’s taking away what’s doing the damage. For a few weeks, stop using acids, cosmetic retinoids, scrubs and fragranced products, cut your cleansing down to a minute and wash with lukewarm water. More often it’s tightness after cleansing, stinging from a familiar cream and grey, rough skin all at once – oily skin included. Before you buy another ‘barrier’ product, check what’s damaging it: how long and how hot you cleanse, hard water, the number of actives and fragrance. And if the signs don’t go away despite gentle skincare, make sure it really is your barrier.

Aleksandra Kubas-Maliszewska

Sources

Misery L et al. People with acne have been found to have fewer ceramides in the stratum corneum and a weaker barrier, and researchers link this to the formation of blackheads and whiteheads. On the other hand, harsh anti-acne skincare weakens the barrier in its own right, so it’s easy to get caught in a vicious circle.

How long does a damaged skin barrier take to heal?
Comfort – less tightness and stinging – usually returns within a few days of gentle skincare, while clear improvement takes a few weeks.
