Skin that feels congested, however carefully you look after it? The culprit may well be sitting on your bathroom shelf. Acne cosmetica – breakouts triggered by skincare and make-up – can appear at any age and on any skin type, often out of the blue. The irony is that it’s caused by the very products that were meant to help.

Products that don’t suit your skin, or simply too many of them, block the pores. Sebum can’t escape, and closed comedones form, sometimes followed by spots and redness. The tell-tale sign: the bumps appear exactly where you apply your products – and settle once you stop using them.

That’s what sets acne cosmetica apart from other types of acne. It isn’t driven by hormones or genetics; it’s your skin’s direct response to what you put on it. But contrary to what you’ll often read, the problem is rarely one ‘bad’ ingredient. Pores become clogged by build-up: too many layers at once, products applied to skin that hasn’t been properly cleansed, or residue left over from your previous routine. An ingredient that’s perfectly fine in a small amount can become a problem once it’s layered with three other products.

In this article, I’ll explain which habits lead to acne cosmetica, how to tell it apart from hormonal acne and other look-alikes, and how to bring your skin back into balance.

Have a question about your own skin? The real cause of acne cosmetica

How to clear closed comedones caused by skincare

What does acne cosmetica look like?

Acne cosmetica most often shows up as small comedones – both open (blackheads) and closed (whiteheads, those tiny bumps beneath the skin). In more advanced cases, spots, red bumps and redness can follow. The giveaway is location: the bumps appear exactly where you apply your skincare or make-up – on the cheeks, forehead and chin, and sometimes on the neck or décolletage, if that’s where your moisturiser or foundation goes too.

A quick note on terminology, because it causes plenty of confusion: a closed comedone is what most of us call a whitehead – a small, skin-coloured or whitish bump with the opening of the pore sealed over. It isn’t the same as a spot with a yellow, pus-filled head; that’s a pustule, which is already inflamed. You may read that closed comedones can’t be popped – and that’s a good reason not to try. Squeezing a sealed pore pushes its contents deeper and can leave a mark.

Acne cosmetica usually doesn’t involve deep inflammation. You won’t typically get painful, hard lumps deep beneath the skin. The bumps are superficial and scattered, and they build up gradually – often so slowly that we don’t connect them with our current routine.

Acne cosmetica or hormonal acne?

This is where a common misdiagnosis creeps in. Acne cosmetica is often mistaken for hormonal acne, especially in women over 25. The difference matters. Hormonal acne tends to sit along the jawline, chin and neck, follows a cyclical pattern – flaring in the days before a period – and brings deep, painful, inflamed spots. Acne cosmetica has no monthly rhythm: it appears steadily rather than in waves, and it settles once you stop using the product that triggered it. I explain the hormonal side in detail in my guide to hormonal acne.

If you’re unsure which type you’re dealing with, a GP or dermatologist can tell them apart and recommend the right treatment.

Fungal acne and other look-alikes

Not every crop of tiny bumps is acne. The most common look-alike is Malassezia folliculitis, often called ‘fungal acne’ – an overgrowth of a yeast that lives naturally on our skin, inside the hair follicles. Despite the name, it isn’t acne at all, which is why it doesn’t respond to typical blemish care. A few clues help tell it apart from acne cosmetica:

the bumps are strikingly uniform – all roughly the same size;

they itch, sometimes noticeably;

besides the forehead and hairline, they often appear on the chest, upper back and shoulders – places where you don’t apply face products;

they flare after sweating, in heat and humidity, or under tight clothing.

Acne cosmetica, by contrast, rarely itches and follows your products: it appears where you apply them.

Other conditions can look similar, too: milia (tiny, hard, pearly-white cysts, often around the eyes), keratosis pilaris (a rough, ‘goosebump’ texture, more typical of the upper arms) and bacterial folliculitis. If your bumps itch, spread, or don’t improve after a few weeks of a simpler routine, get them checked – a fungal infection needs different care, and a simple skin scraping can confirm it.

Skin purging or a breakout?

If the bumps appeared after you introduced a new product, there’s one more possibility: purging. Ingredients that speed up skin cell turnover – retinoids such as retinol, or exfoliating acids (AHAs and BHAs) – can bring to the surface the microcomedones that were already forming beneath the skin. It looks like a flare-up, but it doesn’t mean the product doesn’t suit you.

How to tell them apart:

Only actives cause purging. A moisturiser, a face oil or a foundation can’t make your skin purge – if bumps appear after one of these, it’s a reaction to the product.

Purging shows up where you usually break out. Acne cosmetica can appear in new places – wherever the product goes.

Purging is temporary. It usually settles within around four to six weeks, as the skin completes its renewal cycle. Bumps that keep coming beyond that, or get worse, are a breakout – and the product needs to go.

Acne-prone skin care routine – basic principles

Acne is one of the most common skin conditions, usually affecting people aged 12–24… Read the full article!

Acne is one of the most common skin conditions, usually affecting people aged 12–24. The real cause of acne cosmetica

You’ve probably heard of comedogenic ingredients – mineral oil, petrolatum, lanolin, certain silicones and waxes. They’re flagged as pore-clogging ingredients, and plenty of people scan labels for them. But there’s an important nuance that rarely gets mentioned: the problem isn’t the ingredient itself, but how much of it ends up on your skin, and in what conditions.

Comedogenic ingredients: why the lists don’t tell the whole story

The term acne cosmetica was coined in the early 1970s by dermatologists Albert Kligman and Otto Mills, and the research behind comedogenicity lists was carried out under very particular conditions. In one of the most widely used models, products were applied to a rabbit’s ear, where the follicles are much larger – and far more reactive – than ours. The product went on every day, without the skin being cleansed in between, for several weeks, and only then were the results measured. In another model, testing was done on the human back – where pores are also larger than on the face – with the product held under an occlusive patch.

That’s a world away from applying a moisturiser to your face every day. To a cleansed face.

It’s also worth knowing that ‘non-comedogenic’ on a label isn’t backed by a single, legally defined test. It’s a useful pointer, not a guarantee.

Build-up, not one ‘bad’ ingredient

So what really causes acne cosmetica? Most often, build-up. You apply a product to skin that hasn’t been properly cleansed, with traces of sebum, sweat, dead skin cells and your previous product still on it. Then you add another layer on top, and another. Your skin sheds dead cells naturally, but instead of flaking away freely, they get trapped beneath the layers of product. The pore openings become blocked, Cutibacterium acnes – a bacterium that naturally lives in our follicles – multiplies in the trapped sebum, and blemishes appear. Not because one ingredient is ‘bad’, but because your skin had no chance to breathe*.

*Strictly speaking, skin doesn’t breathe – gas exchange through it is minimal, so this is shorthand. In everyday language, ‘letting skin breathe’ means keeping the pores clear: sebum can reach the surface freely, dead cells shed naturally, and the skin regulates its moisture properly through transepidermal water loss (TEWL). When the openings of the sebaceous glands are clogged by layers of product, that natural process is disrupted – and that is exactly the mechanism behind acne cosmetica.

Don’t forget your hair products

If the bumps sit mainly along your hairline and across your forehead, look beyond your skincare. Hair oils, leave-in conditioners, serums, styling creams and dry shampoo all end up on your skin – via your pillow at night, your fingertips, or simply your fringe resting on your forehead. Dermatologists even have a name for this variant of acne cosmetica: pomade acne. The rule is the same as with face products: it’s about build-up, not a single ingredient. Apply hair products away from the hairline, wash your face after styling, and change your pillowcase regularly.

How to clear closed comedones caused by skincare

Double cleansing, done properly

Proper cleansing is the foundation – and it’s where most of us make the first mistake. If you wear full make-up and sunscreen, a gentle cleansing emulsion on its own won’t be enough. I always recommend gentle emulsions for everyday washing, because they’re kind to the skin barrier, but with that many layers on your face, you need to start with micellar water. It dissolves make-up, even heavy foundation, and copes with sunscreen, which is designed to cling to the skin – so when you cleanse, always treat your SPF like make-up. Micelles work like magnets: they attract and surround oily particles (make-up and the grime that builds up through the day are oily), lifting away everything that has accumulated on your skin. But micellar water alone isn’t enough.

The second step is washing with a gel, foam or emulsion for at least 60 seconds. That’s not an arbitrary number: the cleanser needs time to reach all the residue and clean every contour of your face. A rushed ten-second wash only looks like cleansing. This is double cleansing at its most practical, and only after this step is your skin truly ready for the rest of your routine. As a bonus, washing doubles as a mini massage: it boosts circulation, which supports the skin’s natural renewal and helps dead cells shed more readily. Every week or two, you can also add gentle ultrasonic exfoliation: an ultrasonic skin scrubber lifts surface build-up without rubbing – just don’t use it on inflamed spots.

Less product, one layer

After cleansing, apply a toner or thermal spring water and – this is important – follow straight away with your skincare, while your skin is still damp. The moisture helps the product spread evenly, so you need less of it. If you use more than one product, don’t layer them one after another. Instead, measure out a small amount of each, mix them in your palm and only then apply to your face. That way, you naturally control how much goes on and don’t overload your skin. One layer instead of several – it makes a real difference.

After your routine, your skin should feel satin-smooth: neither sticky nor greasy, and it shouldn’t drag under your fingertips. If it feels sticky, the formula was short on fatty acids, so add a touch of something richer, such as an oil serum. If it feels greasy, you’ve used too much of an oil-based product. Three to four drops of a face oil or oil serum are plenty; combined with a cream, two drops will do. Less really is more: skin doesn’t like being overloaded – neither with active ingredients nor with sheer quantity.

It goes without saying that you should never go to bed without washing your face. In the same way, don’t pile fresh sunscreen on top of what’s already there – apply it to freshly cleansed skin. If you want to avoid breakouts in summer, wash your face before every application.

Want to know the right order to apply your skincare, so it works without weighing your skin down? Read my guide to skincare layering order.

Find the culprit: a simple elimination routine

Rather than throwing everything out, approach it like a detective:

Go back to basics for two to four weeks: a thorough cleanse morning and evening, one moisturiser, and sunscreen during the day. Put extra serums, masks and new launches aside.

Watch your skin. If no new bumps appear, your routine was simply overloading it. The ones that have already formed need a few weeks to settle – don’t expect them to vanish overnight.

Reintroduce one product at a time, roughly every two weeks, and note any changes. Bumps reappearing where you apply it? You’ve found your culprit – or you’re using too much of it.

Remember what else touches your face: make-up brushes and sponges, pillowcases, your phone, hats and headbands.

Products that support blemish-prone skin

You now know what causes acne cosmetica and how to prevent it. But how do you help your skin settle faster? Here are a few products and ingredients I trust to help keep pores clear and support your skin while blemishes settle.

Smoothing Cleanser
A multitasking product that cleanses without disrupting the skin barrier. It works as a cream cleanser, make-up remover and gentle exfoliator in one. Its natural particles are smaller than 100 microns and rounded, with no sharp edges, so rather than scrubbing the skin, they polish it – lifting dead cells without irritation. In an instrumental test, it reduced the number of clogged pores by an average of 39.4% in just 7 days.

Azelaic acid
One of the best-researched ingredients for blemish-prone skin. It helps calm redness, fades the marks blemishes leave behind, helps regulate sebum and limits the growth of acne-related bacteria – while being gentle enough for sensitive, reactive skin. In the UK, higher-strength azelaic acid (15–20%) is a prescription medicine, so speak to your GP or pharmacist; lower concentrations are available in cosmetic products. Use it in the evening, on cleansed skin. You’ll find more in my guide to azelaic acid.

Cream
A hypoallergenic, deeply moisturising cream based on natural plant oils and butters – and yet it won’t clog your pores. It’s whipped, so it’s remarkably light: its density is around 0.56 g/ml, roughly half that of a typical cream. It also adheres to the skin very well, which means you need very little. Apply a pea-sized amount to freshly cleansed, still-damp skin – exactly as described above – and it will support your skin barrier and protect against dryness without weighing your skin down. The ideal base for a minimalist routine while your skin recovers from acne cosmetica.

Discover Pure Raw Rich Then use less product: a small amount, in one layer, on damp skin. Bumps that have already formed need a few weeks to settle.

What does acne cosmetica look like?
Lots of small bumps, clogged pores and sometimes spots, mainly where you apply your products. The surface of the skin feels uneven.

How often should I wash my face if I have acne cosmetica?
At least twice a day, and always before applying moisturiser, make-up or sunscreen.

Can make-up cause breakouts?
Yes, but rarely because of a single product. The problem is usually the amount, or applying it to skin that hasn’t been cleansed. Make-up and sunscreen are the most common culprits.

Is my skincare breaking me out?
If the bumps appear where you apply a product and settle when you stop using it, very likely yes.
